Phyllis J. Ladka

Phyllis J. (Smith) Ladka, 79, of Carlisle, died Friday, January 16, 2015 at the Carlisle Regional Medical Center.
Born April 15, 1935 in Mineola, NY, a daughter of the late Francis J. and Carolyn Amann Smith and the widow of Karol W. Ladka, who died December 24, 2014.
She was a graduate of Glen Cove Nursing School. Phyllis was an LPN with the Central Suffolk Hospital of Riverhead, NY. She was a former member of St. John's Roman Catholic Church, Riverhead, NY. She loved to travel, and especially loved the time spent with her loving family and caring for her pet dachshunds.
Surviving are three sons: Ronald J. (and wife Donna) Ladka of White River Junction, VT, Robert C. (and wife Suzanne) Ladka of Jamestown, NC, and Richard F. (and wife Jacqueline) Ladka of Dodge City, KS; one sister, Carol Lang of Long Beach, NY; eight grandchildren; and one great-granddaughter. She was preceded in death by a sister, Patricia Steele.
Graveside services will be held at 11:00 a.m., Tuesday, January 27, 2015 at Indiantown Gap National Cemetery, Annville with Rev. William C. Forrey officiating. There will be a viewing from 9:30 to 10:00 a.m. at Hoffman-Roth Funeral Home & Crematory, Inc., 219 N. Hanover Street, Carlisle, PA 17013.
